In Vitro Fertilization (IVF) is a modern treatment for couples who have difficulty conceiving naturally. Instead of fertilization (the union of the egg and sperm) happening inside the woman's body, it occurs under controlled conditions in the laboratory – "in vitro".

During this procedure, eggs are collected from the woman's ovaries and fertilized with the partner's (or donor's) sperm in the laboratory. After embryos are created, doctors select the healthiest one and place it in the woman's uterus, aiming for pregnancy.

Who can undergo In Vitro Fertilization (IVF)

Women with fertility problems: Blocked or damaged fallopian tubes, endometriosis, ovulation disorders or low ovarian reserve, advanced age (usually over 35 years).

Men with male infertility: Low sperm count, poor motility or abnormal sperm shape, obstruction in the ejaculatory ducts.

Dr. Genc Kabili completed his medical studies at the "Faculty of Medicine, University of Tirana" with excellent results and then pursued postgraduate specialization in Obstetrics and Gynecology at this faculty.

He has over 25 years of experience in Obstetrics/Gynecology, mainly focused on In Vitro Fertilization (IVF) and infertility treatment in couples.

International Training: Dr. Kabili worked for 3 years at the Geneva Maternity Hospital, Switzerland, where he trained and specialized in In Vitro Fertilization (IVF), controlled ovarian stimulation techniques, Intra-Uterine Insemination, genetic counseling for PGT-A, and Fetal Medicine.

1. Initial consultation and couple assessment

In the first meeting, a detailed interview is conducted with the couple to understand medical history, duration of infertility, and any previous treatment. This is the step where the personal treatment plan is built.

2. Hormonal analysis and laboratory examinations

Hormonal analyses are performed (FSH, LH, Estradiol, AMH, TSH, Prolactin, etc.) to assess ovarian reserve and hormonal status. In the male partner, a sperm analysis (spermiogram) is done to evaluate sperm quality.

3. Transvaginal ultrasound and uterine check

Used to assess the condition of the ovaries, uterus, and endometrium. If necessary, additional examinations such as hysterosalpingography (HSG) or hysteroscopy are performed.

4. Controlled ovarian stimulation

Personalized hormonal injections are used to stimulate the ovaries to produce more eggs than in a natural cycle. The duration is usually 8–12 days and is closely monitored.

5. Follicle and estradiol monitoring

During stimulation, frequent ultrasounds and blood tests are performed to assess follicle development and determine the ideal time for egg retrieval.

6. Administration of maturation injection (trigger shot)

When the follicles are mature, an injection (hCG or GnRH agonist) is given to induce final maturation of the eggs before retrieval.

7. Egg retrieval (OPU - Ovum Pick Up)

Under light anesthesia and with ultrasound guidance, the doctor collects eggs from the ovaries. This is a short procedure (10–15 minutes), performed under sterile conditions and painlessly.

8. Laboratory fertilization of eggs

The collected eggs are combined with sperm in the laboratory for fertilization. In natural cases, classic IVF is used. In cases with sperm problems, the ICSI method (microinjection of sperm into the egg) is used.

9. Embryo cultivation and observation

Embryos develop under controlled laboratory conditions for 3–5 days. During this process, they are carefully monitored at every stage of their development, so that embryos with the best potential for a successful pregnancy are selected. Usually, the embryo is allowed to reach the blastocyst stage for a transfer with higher chances of success.

10. Preimplantation genetic testing (optional)

For couples with a history of repeated failures or genetic diseases, PGT-A (for aneuploidies) or PGD (for specific mutations) can be performed to select the healthiest embryos.

11. Embryo transfer to the uterus

The embryo (or more than one, as appropriate) is transferred to the uterus with a very thin catheter. This is a painless process and does not require anesthesia. The transfer can be done in the same stimulation cycle (fresh transfer) or in a later cycle (Frozen Embryo Transfer – FET).

12. Hormonal support after transfer

After the transfer, hormonal supplements (usually progesterone) are prescribed to help implantation and support the pregnancy in the first weeks. This therapy is essential to create a favorable environment in the endometrium and increase the chances for the embryo to develop successfully.

13. Pregnancy test (β-hCG)

About 10–14 days after transfer, a blood test is done to measure β-hCG levels – the pregnancy hormone. If positive, the first ultrasound is planned to confirm the presence of the embryo in the uterus.

14. Pregnancy follow-up by a specialist doctor

In case of a positive result, the patient is followed by Dr. Genc Kabili, or by other obstetrician/gynecologist doctors of the Egian by Intermedica medical center such as Dr. Arian Kashami or Dr. Jonida Dautllari, during the first weeks of pregnancy to ensure that embryonic development is progressing properly.
